Easterseals Midwest’s Health Home Services aims to connect people with disabilities to high quality, consistent health care. In partnership with the Missouri Department of Mental Health, Division of Developmental Disabilities and Compass Health Network, Easterseals’ Health Home Services provide health management to assist individuals with meeting their health and wellness goals while ensuring they get the care they need.

What is Health Home?

Despite the name, Easterseals Health Home Services does not involve a physical home. Instead, Health Home is a network of healthcare professionals to help address health concerns of people with disabilities and increase their overall wellness. 

Compass Health Network

Our partnership with Compass Health Network provides access to a decade of medical and behavioral health care in offices across the state. Compass' network of nurses and physicians, along with Easterseals' specialized health consultants will make up the Easterseals Health Home team and network.

Health Home Services will:

  • Develop healthcare goals with participants along with a plan to reach those goals and detailed goal tracking. 
  • Provide education on general health and wellness.
  • Assist individuals with understanding and managing their specific health conditions, medications, and treatments. 
  • Access to consultations from healthcare experts. 
  • Keep individuals connected to their current doctor.

Enrolling in Health Home Services and Eligibility

Any Missourian with an Intellectual or Developmental Disability, Medicaid, and one of the following conditions can qualify for Health Home Services: Diabetes, Asthma, Cardiovascular disease or hypertension, COPD, Obesity, Dementia, dependency on a ventilator, tobacco use, diagnosis of autism, Fatal Five Conditions or Health Risk Screening Tool of three or greater
