Health & Attendance Policies

We remain committed to doing our very best to reduce the risk of illnesses and exposure at Easterseals DuPage & Fox Valley. We appreciate your efforts as well.

Masks are now optional but recommended at our centers.

We continue to follow CDC guidelines regarding strategies to reduce the spread of Covid-19 and other infectious diseases and remain committed to enhanced cleaning and disinfecting protocols.

If you prefer that your provider wear a mask when working with your family, please let him or her know.

Stay home and reschedule to tele-therapy if experiencing an undiagnosed rash, symptoms of viral or bacterial illnesses including COVID, colds, flu, gastrointestinal illness, etc. Please click here for CDC guidelines and specific situations to isolate. CDC GUIDELINE UPDATE FOR COVID-19

Asymptomatic CLOSE CONTACT EXPOSURE:

  • No longer need to isolate if you are able to wear a well-fitted mask around others for 10 days.
  • Test on Day 6; Continue masking for remaining 5 days if negative.  If positive, start isolation.

MILD, symptomatic COVID-19:

  • Isolate for at least 5 days after symptom onset and UNTIL fever ends for 24 hours (without the use of fever-reducing medication) and symptoms are improving, May return if able to PROPERLY wear a well-fitted mask around others for another 5 days.
  • Day 0 is the first day of symptoms.
  • If unable to properly mask, isolate for 10 days.
  • If you have moderate symptoms such as shortness of breath, dyspnea, or radiographic respiratory changes, isolate for 10 days.
  • Test as soon as possible after symptoms start if you want to avoid OR reduce need for isolation.

SYMPTOMATIC BUT TEST NEGATIVE FOR COVID-19:

  • Do not return until you are fever-free for 24 hours without medication and symptoms are improving.  You may not have COVID but may have the flu or other viral illness that we do not want to spread.
  • A 2nd test after 48 hours is recommended but not required.

Asymptomatic but test positive (never develop symptoms): 

  • Isolate for at least 5 days after the first positive test. May return if able to properly wear a well-fitted mask around others for another 5 days.
    • Day 0 is the day the specimen was collected for the positive test.
    • If unable to properly mask, isolate for 10 days.
    • If symptoms develop after a positive test, the 5-day isolation period starts over (day 0 changes to the first day of symptoms).

SYMPTOMS MAY include new or worsening:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

Consult with your primary healthcare provider if you have any questions.




REMINDERS

Attendance

Attendance is expected at 85% for you/your child to gain the most from services.  Your provider(s) will work with you on shifting to tele-therapy, flexible scheduling, taking breaks, etc., during times that this is not likely to occur.

  • We require a minimum of 24-hour notice for cancellations.  Please email us at cancel@eastersealsdfvr.org, call the center to leave a message, or fill-out a cancel card at Reception.
  • Same Day cancellations will incur a fee of $50 per scheduled session. 
  • We understand that there are emergencies and illnesses that are unexpected.  Our staff will work with you to avoid fees in these situations including switching to tele-therapy during isolation or quarantine periods.
  • No Shows (any time you have not contacted us prior to the start of a session) will incur a fee of $100 per scheduled session.

Payments

All co-pays and expected balance payments are due at the time of service. Requirements for co-payments are determined by your insurance plan. When required, co-pays must be paid prior to your therapy session and may be completed at the Kiosk, through your Patient Dashboard, or with Reception. You may securely store a payment method to speed the process up if you choose.

Balances are due when you receive your statement and may be paid online, by mail, or in person at Reception using cash, check, or credit card.  Please contact Yolanda DeLuna, ydeluna@eastersealsdfvr.org in Billing if you need assistance or have questions regarding your balance.  Past due balances may result in service interruption until satisfactory arrangements are made.

Communication

You can check your appointment schedule from the Patient Dashboard and if you have opted in, receive appointment reminders.  If you do not receive appointment reminders and would like them, please email portalhelp@eastersealsdfvr.org.  Provide the client’s name, date of birth, your email, and cell number to be set-up in the system.
