Health & Attendance Policies

If your child has any of the below symptoms, please reschedule your appointment with as much notice as possible by email at schedule@eastersealsdfvr.org. Please do not bring family members or others with these symptoms to the Center.

Reschedule if your child has:

  • Upper-respiratory infection, cough or sore throat, and fever.
  •  Fever, gastrointestinal upset-abdominal pain, nausea, vomiting or diarrhea in the past 24 hours
  • Undiagnosed Rash or skin eruptions (must provide written medical clearance with any precautions indicated).
  • Head or body lice.

If a client or visitor is symptomatic, Easterseals DuPage & Fox Valley staff will request to reschedule to teletherapy for that session and until symptoms meet the above CDC guidance designed to reduce the spread of respiratory illness.


Policies

  • While masking is no longer required, we do support and respect the use of masks. If you want your provider to use a mask, just ask! Clients and staff may request the use of a mask by the other during services.
  • Please wash hands before and after services, eating, using the restroom, or any other time your hands may be soiled with bodily fluids.
  • Each client can be accompanied by up to three caregivers and/or siblings at Easterseals. All siblings must be supervised and not left unattended.
  • Please leave bikes, scooters, roller shoes/skates, etc. outside unless your therapist asks you to bring them in for your therapy appointment.
  • Please use our Patient Dashboard for co-pays, check-in and all forms including after visit summaries.
  • Shoes must be always worn (except when the therapist requests otherwise). 
  • Children may not play on or with therapy equipment except during supervised therapy sessions.
  • If you see any area of our facilities in need of cleaning, please notify Reception.
  • No food or beverages should be consumed in therapy rooms except as part of a therapy session.
  • Please do NOT use cellphones in the lobby or therapy rooms. 
  • Please do NOT take photos or videos of your child during therapy without the express knowledge of the provider.  NEVER include other clients or visitors in your photos or videos.


We do not require that you submit vaccination records during admission for therapy services (they are required in the Child Development Center) but we highly recommend that you follow health department guidance. Please understand that we may need to request a copy of records or proof of immunity if we become aware of measles activity in our area or Centers and are advised to do so by the health department. Health Guidance

When sick with a respiratory virus, the updated CDC guidance recommends:

  1. Stay home and away from others while symptomatic. 
  2. Return to normal activities when, for at least 24 hours, symptoms are improving overall, and if a fever was present, it has been gone without use of a fever-reducing medication.

When resuming normal activities, please take additional prevention strategies for the next 5 days to curb disease spread, including:

  1. Enhancing hygiene practices (hand washing, disinfecting high touch surfaces).
  2. Wearing a well-fitting mask.
  3. Keeping a distance from others in public spaces, when possible.
  4. Getting tested for respiratory viruses.
  5. Opening windows and/or using an air filter.



REMINDERS

Attendance

Attendance is expected at 85% for you/your child to gain the most from services. Your provider(s) will work with you on shifting to tele-therapy, flexible scheduling, taking breaks, etc., during times that this is not likely to occur.

  • We require a minimum of 24-hour notice for cancellations. Please email us at schedule@eastersealsdfvr.org, call the center to leave a message, or fill-out a cancel card at Reception.
  • Same Day cancellations will incur a fee of $75 per scheduled session. 
  • We understand that there are emergencies and illnesses that are unexpected. Our staff will work with you to avoid fees in these situations including switching to tele-therapy during isolation or quarantine periods.
  • No Shows (any time you have not contacted us prior to the start of a session) will incur a fee of $125 per scheduled session.
    • If you find you are unable to attend your appointment due to child illness or family emergency, on the day of the appointment, you may click your reminder, answer “no” to would you like to check-in, and then send a message right from the patient dashboard.  Our Scheduling Team will contact you to reschedule.

Payments

All co-pays and expected balance payments are due at the time of service. Requirements for co-payments are determined by your insurance plan. When required, co-pays must be paid prior to your therapy session and may be completed at the Kiosk, through your Patient Dashboard, or with Reception. You may securely store a payment method to speed the process up if you choose.

Balances are due when you receive your statement and may be paid online, by mail, or in person at Reception using cash, check, or credit card. Please contact Yolanda DeLuna, ydeluna@eastersealsdfvr.org in Billing if you need assistance or have questions regarding your balance. Past due balances may result in service interruption until satisfactory arrangements are made.

Communication

You can check your appointment schedule from the Patient Dashboard and if you have opted in, receive appointment reminders.  If you do not receive appointment reminders and would like them, please email portalhelp@eastersealsdfvr.org. Provide the client’s name, date of birth, your email, and cell number to be set-up in the system.

If your child will be escorted to therapy by someone other than a legal guardian, please be sure to sign a Consent to Treat a Minor Child form  consent to treat minor child.docx and a Release of Information  Authorization for use and disclosure of information 03 2018 (1).docx.  These forms are available from your Patient Dashboard or just download, complete, and submit to records@eastersealsdfvr.org or return to Reception at your next visit.  You may also request a copy of the forms from our Reception staff if that is more convenient for you.
