Attendance is expected at 85% for you/your child to gain the most from services. Your provider(s) will work with you on shifting to tele-therapy, flexible scheduling, taking breaks, etc., during times that this is not likely to occur.
All co-pays and expected balance payments are due at the time of service. Requirements for co-payments are determined by your insurance plan. When required, co-pays must be paid prior to your therapy session and may be completed at the Kiosk, through your Patient Dashboard, or with Reception. You may securely store a payment method to speed the process up if you choose.
Balances are due when you receive your statement and may be paid online, by mail, or in person at Reception using cash, check, or credit card. Please contact Yolanda DeLuna, firstname.lastname@example.org in Billing if you need assistance or have questions regarding your balance. Past due balances may result in service interruption until satisfactory arrangements are made.
You can check your appointment schedule from the Patient Dashboard and if you have opted in, receive appointment reminders. If you do not receive appointment reminders and would like them, please email email@example.com. Provide the client’s name, date of birth, your email, and cell number to be set-up in the system.
