Easter Seals Blake Foundation provides accessible transportation for individuals who require door-to-door options and often unable to access public transportation or live where there is no public transportation. This service enhances mobility options for individuals with disabilities and seniors which allows them to independently access their communities throughout Southern Arizona. We work closely with the Arizona Department of Transportation which provides some funding through its Coordinated Mobility Program.
Easter Seals Blake Foundation also provides transportation service to individuals whose transportation needs have been assessed by an Individual Service Plan team, when no other community or family resources for transportation are available, and with prior authorization from the Arizona Division of Developmental Disabilities. We provide non-emergency ground transportation in the following situations: to/from day treatment programs, to/from therapy appointments, to/from home visits, to/from employment, and to access communities for shopping, doctor appointments and recreation. Utilization is generally two one-way trips per day. Service is typically Monday thru Friday 6:00 a.m. - 8:00 p.m.; however, weekend and evening transportation may also be scheduled. Every effort is made to accommodate same day requests, but advance planning is recommended to assure a ride. This transportation program is provided in Pima, Graham, Greenlee, Cochise and Santa Cruz counties.
