Case Management

This is a voluntary service that employs case managers to work with adults diagnosed with mental illness. Our staff communicates with all those important to an individual’s care; including family, mental and physical health care providers, and a variety of community-based programs. Case managers work to develop a plan of care that supports an individual’s goals and preferences. Through case management, we teach participants the skills required to achieve their goals and stay consistent with their treatment.

Through our Supportive Housing & Integrated Case Management Services we:

  • Assess – We work with the individual to identify the condition/needs, abilities, goals, and preferences of the individual.
  • Plan – After the assessment we develop a care plan that defines the strategies and next steps toward achieving the desired outcomes with the ultimate goal of helping individuals take control of their care.
  • Support – A case manager aligns all the moving parts of the care plan and puts them into action along with the individual served. This could include connecting them to mental and physical health programs, vocational services, financial assistance, housing programs, etc.
  • Evaluate – We work with the individual to identify what’s working, what’s not working, and what needs to be changed.
  • Promote – By encouraging the individual served to take control of their care, we boost their confidence and ultimately, their independence.
