What is intake?

Our intake department at Easterseals New Jersey is committed to seamlessly connecting consumers with tailored services in our Behavioral Health programs and Intellectual and Developmental Disabilities services. Option 1:  Behavioral Health Services. This includes:

  • Behavioral Health Home (BHH): Our coordinated care program links Monmouth County residents with an active Medicaid account who are diagnosed with mental illness to suitable medical and psychiatric providers.
  • Integrated Case Management Services (ICMS): Offering comprehensive case management, we connect individuals diagnosed with mental illness to vital community resources and entitlements within their local areas.
  • Residential (RES):Our supervised residential homes provide a supportive environment for individuals with chronic mental illness, empowering them to develop essential life skills for independent living. With various levels of supervision to cater to diverse needs and preferences, including:
    • Level A+: 24 hours, 7 days a week, supervised residences. Allocated to State Hospital Assignments.
    • Level A: 12-hour supervised residences open to community referrals.
    • Level B: 4-hour supervised residences open to community referrals.
  • Supportive Housing (SH): Through community-based case management, we assist individuals diagnosed with mental illness in achieving stability in various aspects of life, including mental health, employment, education, housing, and community engagement. Our services aim to prevent hospitalization or higher levels of care by enhancing budgeting skills, coping mechanisms, employment training, educational opportunities, and socialization, with or without housing subsidies.



Option 2: Intellectual and Developmental Disabilities Services

Our I/DD services cater to adults aged 21 and above diagnosed with an intellectual or developmental disability before turning 21. To benefit from our programs, participants must possess an active DDD number with an assigned budget in place.

These services include :

  • Day Habilitation (DH): Our facility-based program provides moderate support for individuals, focusing on socialization, personal care, soft skills for vocational planning, and recreational activities, with frequent day trips into the community. We follow a person-centered planning approach, prioritizing Individualized Service Plans (ISP) and focusing on outcomes and goals achieved. Transportation is provided.
  • Community Inclusion (CI): Designed for individuals needing minimal support with activities of daily living (ADL’s), our community-based program offers personalized activities within small groups, promoting independence and meaningful engagement. Transportation is provided, with a ratio of 6 participants to 1 staff member.
  • Residential (IDD Res): Our small group homes residing in Ocean County offer activities for adults who need assistance with daily living tasks and community access. Supervised both at home and during outings, residents enjoy private bedrooms alongside shared spaces for dining and recreation. Transportation for medical appointments, shopping, dining, and events is included.
  • Travel and Recreation (Trek Trips): Tailored for those seeking unique recreational experiences with their peers, Trek Trips offers day and overnight trips to various attractions, museums, theme parks, and seasonal events. From ballgames to theatre performances, participants enjoy diverse outings in their community. These trips will have an additional out-of-pocket cost to cover portions that are not eligible for DDD funding.
