The ICF staff work collaboratively with the youth’s medical team to ensure proper medical treatment is maintained. Medication is given in the homes by Approved Medication Assistive Personnel (AMAP) certified staff. Nurse Coordinators are assigned to each home and oversee the youth’s medical care.
An individualized program plan (IPP) is developed by the youth, family, and interdisciplinary team. The youth engage in in numerous community activities of their choice independently and with their friends and family.
