Residential Services: Comprehensive Educational and Rehabilitative Services

Intermediate Care Facilities (ICFs) located in Rochester, NY:

Easterseals NY (ESNY) provides a residential setting designed for individuals with developmental disabilities to provide comprehensive and individualized health care, educational, and rehabilitation services which promotes their functional status and independence. The ICF provides active treatment, a continuous, aggressive, and consistent implementation of a program of specialized and generic training, treatment, and health and related services, directed towards helping the residents function with as much self-determination and independence as possible in their environments.

The residential staff help the youth with daily living skills such as recreation, cooking, cleaning, personal hygiene, money management, social skills, and self-regulation. The ICF treatment team includes the residential and educational teams, including the school psychologist, social worker, psychiatrist, nurse practitioner, school nurse, classroom teacher (special education teacher) and aide, nutritionist, behavior specialists, and related services (speech language pathologist, occupational therapist, and/or physical therapist). The ICFs are located on the Kessler Center campus in Irondequoit and each provide  24-hour on-site supervision and habilitation services by fully trained staff. The ICF staff work collaboratively with the youth’s medical team to ensure proper medical treatment is maintained. Medication is given in the homes by Approved Medication Assistive Personnel (AMAP) certified staff. Nurse Coordinators are assigned to each home and oversee the youth’s medical care.

An individualized program plan (IPP) is developed by the youth, family, and interdisciplinary team. The youth engage in in numerous community activities of their choice independently and with their friends and family.
