Financial Assistance, Payments, and Good-Faith estimates

Insurance

Our facility will file all insurance claims on your behalf, when applicable. Services may or may not be covered by your health insurance plan. You will be responsible for co-payments, deductibles and other non-covered services that your insurance plan does not pay. Although we will assist you through the process, it is your responsibility to know your insurance coverage.

Financial Assistance

Payments are expected when your bills are received. However, if you are unable to pay in full, please call us at 812-479-1411 and ask to speak with the billing department, because we can help. We offer financial assistance to those who qualify and no-interest extended payment plans.

Financial assistance is available to low-income individuals and families. This is based on the number of people in your household and your gross household income compared to the Federal Guidelines. Special circumstances are also taken into consideration (for example, if a client is in multiple therapies, has outstanding medical debt, or insurance is not covering any cost or adjustment). In order to apply for financial assistance, please complete the Fee Reduction Application Form available at this link and return it with supporting documents. If you need help completing the forms or have questions on specific documents, please contact us to determine which are required. 

Please don’t hesitate to call us at any time during your treatment. We cannot help if we don’t know you need it. 

Payments

We accept cash, check, Discover, American Express, Mastercard, and Visa for payment. You can send your check to: Easterseals Rehabilitation Center, 3701 Bellemeade Ave., Evansville, IN 47714. Payments are also accepted over the phone by credit card.

Good Faith Estimate for those who are uninsured or choose to self-pay fees

Under a new federal rule, medical providers must provide a “good faith estimate” of the expected charges to those who are uninsured or choose to self-pay. Details are included in the good faith estimate form.
