We are currently seeking a Bilingual (Creole/English) Service Coordinator for our Early Intervention Assessment Team in West Palm Beach. The Service Coordinator is responsible for coordination of initial meetings to determine program eligibility. The Service Coordinator also ensures all required documentation is completed and services are recommended to meet the needs of the children in the program.
The team travels to services the Palm Beach County area, providing services in the client's homes or daycare.

A snapshot of what you'll do:
- Provide referrals to agencies within the Healthy Beginnings System of Care for early intervention services, including the transfer of eligible cases to the Early Steps Program
- Provide education to families regarding the services available in the community
- Assist families of eligible children in gaining access to services as well as additional support services available in the community
- Assist families in obtaining information on available funding sources
- Participate as a member of an interdisciplinary team while serving as the single point of contact for the family in obtaining the services needed
Qualifications:
- A bachelor's degree or higher with an emphasis in the areas of psychology, social work, health education, interdisciplinary sociology, early childhood, child development or special education (college transcript and copy of diploma are required)
- One year of hands-on experience with young children and families in a teaching, case management, or counseling role, or with individuals with special needs and/or developmental delays of any age in a case management role
- Bilingual (Creole/English)
- Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, to drive for business-related needs (minimum age of 21 and good driving history)
