Case Manager Job Description
Easter Seals Southern Georgia has an immediate Full-Time opening for a Case Manager. Easter Seals is an organization that provides disability services to residents in various locations, offering alternative solutions to their former daily routines. We provide personal care and aid to transition our clients into residential living arrangements. The Case Manager will provide client care and services management to assigned residential clients within specified region to insure optimal client care. They will also be responsible for insuring that all internal and external agency guidelines are met in regards to client care.
Job Duties & Responsibilities:
- Insure all clients receive care that is in compliance with all applicable standards and regulations.
- Work with Property Managers, Support Coordinators, Medical Providers, and other Service Providers to provide optimal client care.
- Train Property Managers and Direct Support Professionals (DSPs) on individualized client care and aid.
- Insure all client rights are appropriately respected and honored as well as report any violations.
- Conduct visits to all properties for scheduled and unannounced review of client care at least weekly and as needed when clients are on premises.
- Conduct client focused team meetings as necessary.
- Insure Health Risk Screening Tools (HRSTs) are updated annually and as needed for changes in diagnosis, medications, physician visits and hospitalizations.
- Serves as liaison between Support Coordinators, staff, and other service coordinators in regards to client care and needs.
- Work with Support Coordinators and staff to develop person-centered goals.
- Communicate client changes such as HRST updates, emergent care and any client concerns to Support Coordinators.
- Meet with clients and residents to insure their concerns are addressed.
- Attend all client meetings such as Individualized Service Plan (ISP), Behavioral Support Plans (BSPs), Addendum Meetings.
- Monitor and review electronic documentation daily.
- Insure electronic client records, medications, and monthly MARS are accurate and accessible to staff.
- Maintain contact with client's families as needed and required.
- Monitor and insure client health, safety, development and quality of care.
- Maintain current client records on-site at each property.
- Update client forms as necessary.
- Review client goal tracking to ensure that all services are provided and documented in accordance with client ISP.
- Review all assessments and insure follow-up on all recommendations made by the regional clinicians.
- Schedule all client medical appointments and attend as needed.
- Insure submission of required documents to various departments as scheduled.
- Assist Regional Manager with intake and evaluation of potential clients.
- Manage all client admission documentation and records.
- Insure client ISP goals are appropriate according to client abilities.
- Work with Nursing Services to insure client medical needs are met.
- Complete Critical Incident Reports as necessary and submit within established guidelines.
- Insure that clients receive supplies through Specialized Medical Supplies (SMS) as outlined in ISP.
- Utilize community and any other resources available to insure all client needs are met.
- Identify, create and submit Exceptional Rate Requests and Renewals as needed for additional funding and to insure appropriate funding.
- Connect client with community resources and natural support system through activities, outings and other events.
- Manage in-home CLS staff including schedules, electronic time cards and staff concerns.
- Review and critique current client records on-site at each property.
- Review Client satisfaction surveys & implement changes when feasible.
- Maintain cleanliness and functional operation of local satellite office.
- Complete continuing education as required for growth and development.
- Follow Health Insurance Portability and Accountability Act (HIPAA) privacy rule to insure client protected health information is secure.
- Maintain knowledge and understanding of Easter Seals Southern Georgia policies and procedures.
- In the absence of the Regional Manager, may assume duties and responsibilities of this position including interaction with Regional Boards, Clients, Families and ESSG staff as well as other leadership responsibilities as appropriate.
- All other duties as assigned.
Essential Functions:
- 24 hours a day / 7 days weekly availability for emergency situations.
- Travel to multiple ESSG properties for client management, meetings and events.
- Ability to lift, push and pull a minimum of 50 pounds.
- Ability to bend, stoop and physically maneuver to properly secure clients and wheelchairs in vans for transport.
- Ability to speak publicly for various groups to advocate for ESSG mission, vision, values and clients.
- Use of technology and office equipment including: laptop, email, smart phone, Microsoft Office Suite, scanner, fax, printer, PolicyStat system, THERAP system, Client Information System (CIS), Health Risk Screening Tool (HRST) and other client records management systems.

- Support clients in personal growth and development while respecting differences.
Minimum Qualifications:
- Bachelor's Degree in Sociology, Psychology or related field of study.
- 2 years professional work experience in residential disability services.
