The Ages & Stages Questionnaires®, Third Edition™ (ASQ-3) is a developmental screening tool that helps you track your child's progress from one month to 5 ½ years old.
- Identify strengths and concerns: ASQ-3 highlights a child's developmental strengths and areas that may need attention
- Easy to use: Complete the questionnaire in just 10-20 minutes
- Inform development: Get a snapshot of a child's development in key areas like communication, motor skills, problem-solving, and social skills
- Guide next steps: If there are potential concerns, ASQ-3 helps parents and professionals decide on the next steps for support and intervention
For the best experience, please take the screening on a computer, not a mobile phone.
The ASQ results will help identify if your child is developing without concerns or if your child has trouble with some skills. Easterseals, health professionals, early intervention programs, and teachers can help identify next steps to ensure your child gets additional help if needed.
You Have Support
The first five years of a child’s life are a time of remarkable growth and development. We understand how important this period is for laying the foundation for future learning, relationships, and independence.
Easterseals is here to help you navigate this journey, offering resources, guidance, and services tailored to meet your child’s individual needs. Together, we can help make the most of these critical early years, ensuring your child has the tools to thrive now and in the future.

ASQ-3™ is designed for screening, not diagnosis. If there are potential delays or issues, you can get support for your child now when it makes the most difference. Your results from the questionnaire will be emailed to you within two weeks. For children ages six or older, please discuss their development with your child's health care provider, local school district, or your child's teacher. Continue to Track Your Child’s Growth Regularly check in to track your child's development from one month old to their fifth birthday.
