Summary:

For over 25 years, Easterseals Rehabilitation Center's Community Living Services program has provided direct care in individuals' own homes, as well as community access, helping people with disabilities enjoy the best possible quality of life and achieve their personal goals. This position is hourly, working from clients' homes, and some work in the administrative office.

Basic Responsibilities:

Work as a team leader in the clients' homes to provide services and continuity of care while assisting QDDPs with maintaining sites and rotating on-call phone. Exhibit leadership, commitment and teamwork, and assist or supervise clients with the following activities:

  • Preparation of meals
  • Bathing
  • House work
  • Appointments
  • Shopping
  • Recreational activities
  • Medication administration
  • Feeding
  • Dressing
  • Mobility
  • Becoming independent with all living skills
  • Transporting of clients
  • Attending or scheduling of medical appointments
  • Monitoring the safety of the clients

Qualifications:

Required:

  • Have a Bachelor’s Degree, high school diploma, or GED.
  • Have/Maintain First Aid and CPR certification.
  • Complete State Mandatory Medication Administration Training upon being hired. 
  • Have at least one year of experience working with persons with developmental disabilities or complete additional training upon being hired. 
  • Have a valid driver’s license and insured vehicle. 
  • Be at least 18 years of age. 
  • Requires the ability to hear, see, talk, read, and write on constant basis.
  • PHYSICAL REQUIREMENTS: Physical lifting and/or transferring of certain clients is required. Must be able to lift 40 pounds frequently and 80 pounds occasionally.
