This specialized Dementia Clinic will be held monthly to aid in the evaluation of dementia, assess for functional activities of daily living skills, and to provide resources to patients and their caregivers.
This is a separate service from our more comprehensive neuropsychological services. This program is aimed to decrease the amount of time that patients wait to be assessed in order to receive recommendations and treatment for this debilitating disease.
This short and focused evaluation involves a clinical interview with patients, caregivers and/or informant source and standardized assessment addressing areas of attention, memory, executive functioning, social functioning, processing speed, daily living skills, severity etc.
A brief report will be provided to referring providers and families within 2 weeks of the patient completing their evaluation.
