In this position you will be responsible for coordinating all services related to individuals served being assessed for HOME Choice services and transitioning from nursing home to community living, services provided in Hamilton, Butler, Warren, Clermont, Clinton, Highland, Brown, and Adams counties. Responsible for assessing all accepted referrals for engagement in HOME Choice Transition within 7 – 10 days of receipt of referral, assess and develop supports for income, daily living skills, medical care, employment and proper support networks, providing assistance to Home Choice staff, assisting with planning and documentation as well as locating housing of individuals served; Section 8 housing, subsidized senior housing. Assists in transporting individuals to tour apartments, apply for food stamps, social security and other pertinent needs. Assists and/or transports individuals to purchase goods and services for their apartment set-up, assisting with assembly and moving.
Minimum qualifications include:
Knowledge of intervention strategies; emotional, mental and physical disabilities; general principles and methods for working with all populations, knowledge of housing resources, housing application process, Medicaid, Social Security.
Exhibit flexibility in work schedule and job tasks
Develop and maintain effective working relationships with associates, supervisors, and individuals served; maintain and promote good public relations; maintain confidentiality of sensitive subject matter
Valid driver’s license & insurance & a good driving record
