Summary:

To provide comprehensive care to families during transitional periods from the start of services through discharge.  The care coordinator will work with the family to understand the families' priorities, to help identify needed resources and supports, both internal and external to our center, and empower families to obtain resources that best meet the needs of their child. 

Basic Responsibilities:

1. Oversees client transition from pediatric care to adult service model.
2. Facilitates transition discussions with caregivers at client age of 2, 14, 18 and 21.
3. Provides recommendations and education of internal and external services available to client at each transitional period.
4. Facilitates ongoing involvement of client served in the therapeutic process and assists client/family in understanding needs and in making informed decisions.
5. Facilitates multidisciplinary communication and review of progress in addition to coordinating and leading team meetings as needed.
6. Promotes program responsiveness to the strengths, abilities, needs and preferences of the client/family through both formal meetings and informal communication with the family and therapy team.
7. Disseminates data and information regarding the client i.e. school staffing, meetings, medical reports, and other critical information that facilitates integrated service delivery, when indicated.
8. Facilitates the implementation of discharge/transition recommendations.
9. Fields inquiries from families seeking resources and guidance.
10. Oversees parent mentoring program
11. Facilitates and/or identifies both internal and external parent education opportunities.
12. Documents case management activities in the electronic medical record.

Qualifications:

  1. Bachelor’s Degree in Healthcare, social work preferred
  2. Minimum 3 - 5 years of experience preferred.
  3. Maintain current CPR certificate.
