Ohio HOME Choice

HOME Choice is a transition program. It helps you move from a long-term care facility into a home or community-based setting where you will continue to have access to the same services you received in the long-term care facility. Your transition from the long-term care facility into your new home is carefully monitored by a transition coordinator. Your transition coordinator will work with you and your family to help you find appropriate housing, set up your household (acquire furniture and other household goods), connect to community services to help you maintain your independence, and coordinate your health care and prescription services/benefits.

HOME Choice provides participants a greater choice and control over the services received in their preferred setting.

Who is eligible to participate?

To participate in HOME Choice you must:

  • Have lived in a facility-based care setting for at least 90 days
  • Have certain care needs
  • Be eligible for Medicaid

Those eligible may enroll on a state waiver program (programs that provide an alternative to institutional care) or receive State Plan Medicaid services. Also, eligible people may receive community transition services (goods and services that will assist a successful transition to the community) up to $2,000 per person, independent living skills training, a community support coach, nursing, social work or counseling, nutritional consultation, communication aids, and service animals.

Where will I live?

The HOME Choice program moves eligible long-term care facility residents into approved home and community-based settings. Your transition coordinator will help you find appropriate housing. Approved housing options include:

  • A home owned or leased by you or your family members
  • An individually leased and lockable apartment rented by you or your family members
  • A community-based residence

What does the transition coordinator do?

  • Complete intake, obtain abuse and mental health assessments, and secure necessary consents from the individual and/or their guardian
  • Assist with securing housing, keeping in mind the wishes and choices of the individual
  • Verify that housing meets the definition of a qualified residence
  • Participate in discharge planning from institutional setting and assist with development of a transition plan
  • Assists individual in determining most effective use of HOME Choice Goods and Services Funds
  • Assists the indivdual in locating the community resources and referrals such as a physician, pharmacy, etc.
  • Complete follow up on individual for 90 days after transition to the community.
