VA REACH - Northern Region Community-Based Crisis Services

The REACH program has a team of Bachelor level, Master Level, Licensed and Licensed-eligible staff who provide community-based services, which typically include:

  • Responding to 24/7 Crisis & Referral calls
    • Face-to-face crisis response within 1-2 hours based off location
    • Phone consultation with a trained clinician
    • Informational calls
  • Stabilizing the immediate crisis situation
  • Assessment and determine program eligibility
  • Developing Crisis Education and Prevention Plans
  • Training systems on individualized CEPPs
  • Create staffing plan and service plans for the provision of mobile crisis supports
  • Training and Mentoring
  • Linking to service supports
  • Updating and training on prevention strategies


Mobile Crisis Supports:
REACH is able to provide community based crisis stabilization supports in the individual’s natural setting for up to 15 days based off of clinical need.
