Case Management

Easterseals provides traditional Case Management services for Michelle P (MPW), Supports for Community Living (SCL), and Home and Community Based (HCB2) waivers. Case Managers will assist in initial waiver plans of care to access supports and services, as well as existing waiver cases to continue monitoring and accessing services. A client will choose a case manager, who will conduct monthly visits and monitoring of services as required by state regulation.

The main goal of case management will be to work closely with and advocate for the person to ensure they are satisfied with their services and their needs are adequately met. The health, safety, satisfaction of services, and welfare of each person we serve is our ultimate priority.  Case managers can also assist in application for waivers and in locating and accessing community resources.

Easterseals West Kentucky wants to make sure that each individual is receiving services they want to receive and that are appropriate for their needs. We want each individual to receive services that help them grow into being as independent as possible in their lives, while living a truly fulfilling life. They get to define what fulfills them, and we help make that happen.
