POLICY STATEMENT:

Easter Seals Rehabilitation Center grants equal access to its programs and services to all citizens. Transitional Services Specialist, Full Time, 40 Hours/Week

Under the direction of the Manager, Community Inclusion Program, is responsible for providing direct services to individuals enrolled in the Transitional Services program. The Transitional Services program provides career discovery and career exploration opportunities to individuals in need of developing skills in the pursuit of future employment. Transitional Services assist individuals to prepare for participation in the workforce including acquiring or improvement of life skills necessary to be a successful participant in community based employment. Transitional Services start the day and end the day at the work center but the majority of services take place in community settings. Works closely with Placement Specialists in coordinating community based work experiences for individuals enrolled in program. When covering the desk, they will also provide assistance with supportive clerical functions for the receptionist and the audiology department. Under the direction of the Director of Employment and Community Services, is responsible for the day-to-day operation of the Community Services department including oversight of all job development, placement, and case management services for all individuals served and department staff. Works collaboratively with the Vocational Rehabilitation Program Manager and Community Inclusion Program Manager to ensure that the case management needs of all individuals served in the Pre- Vocational, Group Supported Employment, Individual Supported Employment, Day Support Options, and Transitional Services are met. The Director, Employment and Community Services is responsible for all day to day operations, compliance and regulatory activities, quality improvement oversight, and growth initiatives. Works collaboratively with Easterseals leadership team. Fosters excellent organizational communication, employee engagement, and commitment to the community we serve.
