BASIC RESPONSIBILITIES

This position will provide quality physical therapy evaluation and family education and coaching to maximize the functional independence of children birth to three within their families' daily routines and activities. The therapist will be a member of a multidisciplinary early intervention team supporting infants and toddlers with developmental delays and their families in the child's natural environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Direct Services:

  • Assist in the identification, assessment, and intervention for children referred to and/or receiving AzEIP services.
  • Evaluate children referred to the Arizona Early Intervention Program using established evaluation procedures and standardized assessments.
  • Conduct observations in the child's natural environment or least restrictive setting.
  • Ensure that families receive accurate information in a timely manner.
  • Ensure families receive accurate explanations of the child's assessment and that they understand the scores.
  • Communicate clearly in understandable language and without use of unnecessary jargon.
  • Develop and provide coaching for family members to promote the child's development and encourage the child's participation in home and community activities, as part of the Individualized Family Service Plan (IFSP) team.
  • Participate on multidisciplinary team, acting as team lead and/or service coordinator, as appropriate.
  • Provide program services effectively and in a manner that reflects understanding of the centrality of relationships in supporting the child and family growth and development.
  • Develop services that address the functional needs of the child to improve the child's ability to perform tasks in home, schools, and community settings.
  • Provide and accept colleague to colleague coaching and other agency programs when appropriate.
  • Instruct family members in the proper and safe usage of adaptive equipment and assistive technology.

    BASIC RESPONSIBILITIES

    • Attend staff meetings, agency and ECI team in services and trainings.
    • Complete continuing education needed to maintain certification/licensure and renew certification/licensure as necessary.
    • Attend and participate in ECI team related quality assurance meetings and reviews including Utilization Review and Peer Review.
    • Process referrals including all related paperwork, documentation and interagency communication.
      Conduct intakes and screenings per policies.
    • Initiate and maintain contact with children’s physicians for obtaining and updating necessary medical information.
    • Perform all duties related to service coordination including planning, attending and facilitating meetings between families, ESCT staff and other community members as requested by families.
    • Provide routines-based Physical Therapy within qualitative and quantitative standards of the agency.
    • Present in-service training on topics within his/her own discipline or area of expertise.
    • Provide resource information to staff members and families.
    • Assist and facilitate the work of team members, other departments, and agency managers in attaining the goals and objectives of the organization.

    QUALIFICATIONS

    • AA in Applied Sciences, current Texas license in physical therapy, prefer background in pediatrics.
    • Knowledge and skills related to achieving successful outcomes for children and families challenged by developmental risk and/or disabilities in a routines-based, primary service provider model.
    • Demonstrate ability to work cooperatively with other employees, clients and their families, and other customers of the Center.
    • Demonstrate ability to communicate effectively, verbally and in writing, with customers, suppliers, co-workers and supervisors.
    • Work independently and use good judgment.
    • Maintain consistency with administrative and departmental policies through appropriate behavior, dress, attitude, attendance, confidentiality, professionalism and reliability.
    • Knowledge and use of proper handling techniques.
    • Computer knowledge including word processing.
    • Knowledge about agency services, programs, policies and standards.
    • Adhere to safety, health and regulatory requirements as described in the Center’s policies and procedures.
    • Ability to organize, allocate time, and manage caseload requirements.
    • Respect for the value, potential and dignity of service participants and their families.
    • Must be able to lift children up to 30 lbs and sit on the floor for extended periods.
