The Transitions program helps individuals prepare for the next phase of their life, whether they are just leaving high school or simply striving to increase independence through education and employment.
Transitions is a day program based at Easterseals Arc's Coldwater Road facility. However, participants go far beyond our building. All groups go on community outings at least three days a week, with some groups out and about every day Monday through Friday.
Participants complete 17-week rotations in three areas of focus:
Hours: Transitions is available from 7:30 a.m. to 5:30 p.m. Mondays through Fridays (except major holidays).
Participant-to-staff ratios: Our groups have ratios of 10-to-1 or 4-to-1 based on the needs of the participants.
Transportation: The parent/guardian is responsible for providing transportation to and from the program. We can give you information regarding transportation services.
Medical Care: All staff are trained on first aid and dispensing medication and are CPR certified. Also, each staff member is trained on each clients specific information. Nurses and a nurse practitioner are accessible in emergency situations.
Employment Readiness Academy: All Transitions participants who have a goal of community employment have the opportunity to apply for our Employment Readiness Academy. This is a unique program that combines classroom instruction and hands-on work experience in an 18-week paid internship. The Academy began as a partnership between Easterseals Arc and Lutheran Hospital, but has been expanded to include internships at SIRVA, Ivy Tech Community College, and St. Joseph Hospital with additional sites in development.
