About Supported Living

In comparing our supported living program to other programs or providers, here are some things to consider:

  • Usually one to four individuals live in a house or apartment, and we provide direct support professionals to assist with life skills, such as taking medication, grooming, cooking, cleaning or handling finances.
  • In addition to providing support in individuals’ homes, our team ensures that participants in our supported living program have opportunities to pursue their personal interests and take part in local, regional and out-of-state activities and trips.
  • To be eligible for supported living, individuals must have a Community Integration and Habilitation (CIH) waiver and require assistance with health, safety and welfare needs; taking or obtaining medications; all or some daily living skills; transportation and medical appointments; and finances and budgeting.
  • Around-the-clock support means questions can arise at any time. Our call center provides answers 24 hours a day, seven days a week. Anyone —program participants, family, staff, nurses — can call the center at 260.969.4254 for help with a wide range of concerns including medication questions, transportation questions, schedules, and more.
  • We offer a representative payee program for individuals who need help managing day-to-day finances.
  • When transportation is required, staff use their own vehicles. We check staff driving records upon hire and annually to ensure they maintain a clean driving record. Staff also must provide continuous proof of auto insurance. If an individual requires the use of a lift van, the staff will check out an agency van.
  • All staff are CPR-certified and trained on first aid and dispensing medication. Also, each staff member is trained on each individual’s specific information.
