“Never doubt that a small group of thoughtful, committed people can change the world. Indeed, it is the only thing that ever has.” Margaret Meade
Imagine longing to get your independence back and not knowing who to turn to. Unfortunately, many individuals experience this, including Malisa.
In January of 2017, Malisa was rushed to emergency with an infection in her foot. She needed emergency surgery, where she lost some of her big toe. Needing assistance, Malisa was admitted into a nursing home.
This is where she heard of Ohio Home Choice through the Social Worker at Beeghly Oaks. In February, she started working with Amanda, Easter Seals Transition Coordinator, to begin the process.
Malisa stated that, “Having the goal of having my own place, pushed me to work harder in therapy.” If she did not have this hope, she would probably not have done as well as she did. In October, Malisa moved into her new apartment through Easter Seals Ohio Home Choice program. Malisa transitioned with Patriot Home Health to ensure she would continue to have success. Amanda continues to monitor and help Malisa with any questions or concerns she may have. Malisa is very content and wants everyone to know that “Amanda is so amazing and just great!”
Ohio's Home Choice program transitions eligible Ohioans from institutional settings to home and community-based settings, where they receive services and support at home and in their communities.
To participate in Home Choice, you must:
Malisa continues to get settled into her new home. She enjoys crocheting and coloring, and is also working on her health to remain independent. Amanda and Easterseals will continue to offer support to her on her journey.
