Support Coordination

Easterseals Louisiana provides Support Coordination for Louisiana's Medicaid Waiver Program for individuals with developmental and age related disabilities in 5 regions in Louisiana. Chosen by Waiver Participants through a Freedom of Choice, Support Coordinators work with participants to develop a participant centered plan of care, choose service providers, and ensure the authorization and quality of ongoing supports.

EarlySteps SPOE

EarlySteps services include screening and assessment and coordination of early intervention for children with developmental disabilities. ESL plays a pivotal role in this process by serving as the System Point of Entry in two regions in the state. ESL screens referrals and works with eligible families to develop a plan of care and select a family service coordination agency.

Mental Health

ESL offers a number of programs for individuals in need of mental and behavioral health supports including peer support, case management, re-entry, and crisis intervention.

Transitional Housing

Easterseals Louisiana supports individuals experiencing homelessness with a continuum of housing programs including street outreach, respite, rapid re-housing, and transitional housing. ESL utilizes a housing first approach to provide a foundation for wrap-around supports to support clients in moving toward permanent housing.

The Chartwell Center

The Chartwell Center is part of Easterseals Louisiana working to make a life of independence and purpose possible for children and adults with Autism. The Center currently provides Applied Behavior Analysis (ABA), academic services, and vocational training.
