Workforce Development Specialist, Community Link

Easterseals Serving Greater Cincinnati

Individual in this position provides direct services to participants by greeting individuals assigned to Community Link and determining intensity of services needed through intensive interviewing, observations, self-report, standardized test results and reports from other professionals and assists participants in making the best possible work assignment decisions, making assignments to work activity to be incorporated into the community Link Case Plan, referring qualified applicants to approved training and/or other available services in accordance with their Case Plan, ensuring linkage with Work Site Coordinator. Completes necessary documentation regarding assessment, referral and other follow-up services, attends joint case conferences as necessary. Team Lead, Clifton Hub

Individual in this position will be responsible for coordinating services provided to individuals with disabilities in an innovative community based employment program preparing participants for integrated community jobs and recreating/leisure activities. Schedules and oversees daily program activities and provides direct service support to Hub program participants. Serves as day to day liaison with community partners. Individual in this position will be responsible for establishing and maintaining activities to locate and secure job openings for Community Link & Worklink individuals served by contacting employers to develop employment opportunities, researching and maintaining current job listings and developing resources for individuals served to identify current job openings related to their identified career goal. Serves as advocate and liaison for individuals served to utilize employer incentives designed to help applicants on public assistance. Community Support Coach HOME Choice

Easterseals has a part-time position as a Community Support Coach with our HOME Choice program, in this position you will be responsible for implementation of Community Support Coaching and Independent Living Skills Training service lines of the HOME Choice program. These services provide support for HOME Choice participants who have or will be moving back into the community after at least a 90 day stay in the hospital, rehab facility or nursing home. Responsibilities may include assisting with food stamp applications, Social Security benefits, health insurance correspondence, budgeting, and transporting individuals in the community, finding community resources, organization skills and other various tasks and supports to empower successful community living.
