    We recommend: 

    1. Calling your insurance company directly and asking for details about your plan benefits before starting therapy.
    2. Finding out if your providers are in network with your insurance plan using the provider tax ID (36-2476388) ahead of time.

    As a courtesy we will still bill your insurance whether it is not listed and/or you have out of network benefits.

    Please call (630.282.2028) if your insurance is not listed or if you have any questions.

    Financial Assistance

    As a non-profit, we greatly depend on our supporters who enable us to provide financial assistance for children in need. Though the wait list and availability is limited, we remain one of the few health organizations in the area that accept Medicaid. We hope you will join us in continually advocating for this important need by reaching out to your local government representatives. Please help them understand the importance of our critical services and the need to serve more families on Medicaid.

    If you have a concern regarding funding for services, please contact Client Account Coordinator, Yolanda, at ydeluna@eastersealsdfvr.org or 630.282.2028 and receive a Financial Assistance application.  We have financial assistance for medically complex children, children with Cerebral Palsy and additional opportunities through special partnerships. If seeking assistance, a Financial Assistance application must be filled out first so that we can be sure our donated and restricted funds are being directed to those most in need.

    If you need additional resources or are experiencing a hardship, please contact our social services team at mentalhealth@eastersealsdfvr.org. We can connect you to resources for food, clothing, shelter, substance abuse, domestic violence, transportation, internet services, childcare and more important needs. Please don’t hesitate to ask for help. We are your partners to ensure your family is healthy, happy and safe.
