You can use your practice’s form or the Easterseals form available by clicking here
Upon receipt of the referral form, and an Easterseals coordinator will reach out to the family for in-take and information gathering. Once your patient receives their initial evaluation, Easterseals will send a follow-up report to your office.

Insurance and Access Information
Easterseals accepts most major commercial medical group insurances and out-of-pocket payers.
Some services and clinics are available for patients with Medicaid.
As an Illinois Early Intervention Provider, children birth to three have access to services at Easterseals at no cost.
