Service coordination includes gathering social and demographic information, working with the P.A. department to determine the cost of and insurance coverage of Easter Seals services, and then following up to ensure completion of evaluations.
The service coordinator is responsible for developing an individualized plan for each child and family receiving services at Easter Seals. Developing the plan is a team effort conducted collaboratively by parents and service providers. The purpose of the plan is to guide services while representing the family’s priorities.
In addition to coordination services for children at Easter Seals, your coordinator can facilitate referrals to school and community programs.
For those children enrolled in the Early Intervention Program, we coordinate services with the Child and Family Connections office.
