Adult Program Funding

Easterseals Adult Program enrolls clients 18 years of age and older who have a disability that is attributed to a developmental delay, autism, cerebral palsy, epilepsy, or is demonstrated by other major functional skill deficits. Upon admission into the programs, clients must be linked to a pre-admission screening (PAS) agency and be receiving 31U (Developmental Training) or home-based support services through the Department of Human Services.  Referral packets should be sent through your PAS agency to 312-432-9168 or email

If you are not currently linked to a pre-admission screening (PAS) agency, please visit the Illinois Department of Human Services website and follow the steps outlined below:

  1. Scroll down to mid-screen and find the DHS office locator box
  2. Select developmental disabilities from the dropdown box
  3. Select the county in which you currently live
  4. Click on the find offices button
  5. Call the number provided and explain your needs to the intake coordinator
