The applicant must have a disability that is attributed to a developmental delay, autism, cerebral palsy, epilepsy, or is demonstrated by other major functional skill deficits in the areas of self-care, receptive and expressive language, learning, mobility, self-direction, capacity for the independent living and economic sufficiency.
The applicant must be at least 18 years of age at time of admission.
The applicant must present with an appropriate funding source.
Clients who receive a Medicaid Waiver from the State of Illinois Department of Human Services, Division of Developmental Disabilities for Day Program, CILA (inclusive of Day Services) or Home Based Services (HBS) can use their waiver to receive services from Easter Seals.
The admission procedure includes a personal interview, review of the admission referral package materials, a current (within 6 months) physical examination report identifying health limitations, and a psychological report indicating adaptive functioning level. Upon review of a complete intake packet, the multi- disciplinary team will invite the client, parent/guardian, and, if requested, other members of the client’s team or previous team for a personal interview. The interview will include a get-to-know you activity with the Program Coordinator and client, including greetings, questions, and playing a game. There will also be an opportunity for the parent/guardian and Manager of Adult Autism Services to discuss the program and client’s needs.
