Individualized Performance Plans (IPP) are developed to ensure high-quality, person-centered programming. Our program activities are developed with individual client goals, strengths and interests in all domains.

The Adult Program is distinctly unique because we are a smaller, intimate program that ensures person-centered planning and quality evidence based supports for each client we serve. We are accepting referral packets through pre-admission screening agencies at this time.  Please fax referral packets to (312) 432-9168.
